Federally Qualified Health Centers (FQHC)
Federally Qualified Health Centers are community-based health care providers that receive funds from the HRSA Health Center Program to provide primary care services in underserved areas. They must meet a stringent set of requirements, including providing care on a sliding fee scale based on ability to pay and operating under a governing board that includes patients.
Federally Qualified Health Centers may be Community Health Centers, Migrant Health Centers, Health Care for the Homeless, and Health Centers for Residents of Public Housing.

1. Medicare Services (learn more) is a federal insurance program that provides health coverage to eligible enrollees. Beneficiaries must pay monthly premiums, annual deductibles, and coinsurance for many of the services they receive. Some enrollees even have to pay a premium for Medicare Part A, increasing the total cost of coverage. For Medicare beneficiaries with low incomes, it can be difficult to absorb these out-of-pocket costs.
2. Medicaid (learn more) – If you have limited income and resources, you may qualify to get help from Medicaid to pay premiums and other Medicare expenses. Medicaid is a state and federal financial assistance program for people of all ages. If you qualify for Medicare and Medicaid, you are considered dual-eligible.

New Jersey FamilyCare / Medicaid Long Term Care Programs, Benefits & Eligibility Requirements (Learn more) Medicaid is a joint federal and state program, so its rules, coverage plans and even its name all vary by state. This article focuses on New Jersey Medicaid Long Term Care for seniors. This is different from regular Medicaid, which is for low-income people of all ages. In New Jersey, Medicaid is also called NJ FamilyCare and it is administered by the New Jersey Department of Human Services, Division of Medical Assistance and Health Services. Medicaid will help pay for long term care for New Jersey residents in a nursing home, their home, the home of a loved one, an adult family care home, a comprehensive personal care home or an assisted living facility through through one of three programs – Nursing Home / Institutional Medicaid, Home and Community Based Service (HCBS) Waivers or Aged Blind and Disabled (ABD) Medicaid.

Need Help 2-1-1 Help Hotline – This is a 211 Hotline information program set up for the (by United Way) State of New Jersey. This phone number serves just like the regular 411 telephone information phone number. You call this number and tell the operator what social service you need (i.e., food, clothing, furniture, drug treatment, shelter/housing, etc.) and the operator will provide you with a list of agencies and phone numbers that provide the requested services in the requested city. You can also go online to access these services http://www.nj211.org and print out the list of agencies provided.
